About This Role

The General Practitioner will provide clinical leadership within integrated Chronic Care Teams, working alongside allied health professionals and specialists. The role involves a structured two-day work week, consisting of one day in-clinic at their Ipswich partner practice and one day of remote telehealth case-conferencing. You will be responsible for translating clinical findings into coordinated, evidence-based care plans for patients with complex health requirements.

Eligibility Criteria

  • Degree: Medical Degree (General Practitioner)
  • Experience: Experienced practitioners required
  • Must be capable of delivering structured multidisciplinary case conferencing
